Juanita Hernandez

Juanita has been working with the community for many years as a family engagement coordinator. She is excited to bring her knowledge, insight, and experience into her new role as a Community Health Navigator (CHN). Her role is to help community members learn about and connect with helpful mental health-related resources available to them. As a bilingual CHN, she is passionate about working with underserved Sonoma County residents and helping them access services that will improve their quality of life.
